For my website, I am going to:

1.Have pages:
           a. Home
           b. Hospital
           c. Terms of Use
           d.Video

Home:
     I want this page to have an introduction of a greeting to the patient and also an app they can click on and
     a form that pops up to fill out with all their medical information. At the bottom of the page it will have a
     feedback box that will allow the patient to enter any concerns, suggestions of improvement, or comments
     about the service. 

Hospital:
     I want to make this page all about which hospitals are in the Dallas area and give each form for those
     hospitals.

Terms of Use:
     This will be the part that tells the patient that they cannot rome through the internet other than that fill out
     form

Video:
     This will have my video link and it will have a couple paragraphs talking about it.
